Patient Forms
You can save time at your first visit by
printing out and completing these patient forms in advance of
your appointment. Please bring these completed forms to your
first appointment. It is important that you fill out these forms
accurately so we can best serve your dental
needs safely and effectively.

non-prescription medications you are taking at the time of the visit.

First Visit
You
will be greeted by our Patient Care Coordinator who will take you on an
office
tour to meet our staff.
Next our dental assistant will review your forms in the
comfort of our
consultation room. Dr. Peterson will than provide a
personalized session. You will be accompanied to the
treatment room where:

Make a list of
questions and concerns to ask our office, so you don't
forget anything on the day of your appointment. This
includes any concerns you have, or oral problems you've
been experiencing. |

Review your
medical and dental history |
 |
Clarifying your
concerns, wants and expectations and/or problems |
 |
A series of diagnostic
X-rays
will be taken to provide a detailed radiographic
view of your teeth and bone condition. |

Oral Cancer Screening-
the insides of your cheek, your gums, the sides, top and bottom of your
tongue, the roof and back of your mouth will be examined for normal and abnormal
sores or lesions.

Hard tissue Exam- for any
cavities
using:
 |
Diagonodent -a simple laser diode tool used to
inspect your teeth as a means of detecting cavities so
your exam can be fast and easy |

Complete Periodontal Exam-the
gums and bone that surround your teeth will be evaluated for signs of
hidden gum disease. |
 |
Comprehensive
dental evaluation
is recorded in your
tooth chart
showing your existing and needed dental work.

Tour of your mouth with
a miniature
video camera at high magnification. |
 |
Treatment Plan
which is a
written explanation of the complete findings about your
dental health. We
encourage you
to ask questions
because your input into your treatment plan is very important.

Verbal and written
educational materials
are provided based on your dental needs.

"Cleaning”
of your teeth may be done depending on the results from this exam. Dental
cleanings are done by our qualified staff members. |

After your examination Dr. Peterson will review all finds; discuss your
treatment recommendations and sequence; give you a
customized written report and answer all question to your
satisfaction and understanding
at your first visit or at the no fee
consultation, depending on the complexity of the required treatment. |

treatment and
financial
options and
our fees.

Emergency
If you have a dental problem that needs
immediate attention we will see you during our emergency hour
by providing a
Limited Exam and necessary x-rays. We will take time to discuss our findings
with you, show you what we find, answer questions you may have and help you set
up your next appointment for a comprehensive exam.
You will be asked to fill out the forms provided above to
make available pertinent information about your health.
Please bring your
insurance card and your
insurance booklet that describes your personal coverage.

include a thorough examination of the mouth for skin lesions, canker sores,
ulcers and rashes--any of which could be indicative of a more widespread
illness.
"Literally
anything that can happen in your body can happen in your mouth.
It's all made of the same
stuff: blood, lymph, muscle and skin. Both the patient and dentist have a
responsibility to view the examination of the mouth as part of a broader picture
of overall health, and not simply a check-up of an isolated region.
